Please be reminded that applications for CPD Hours by organizer or individual must fulfill the following requirements :

• Use the correct form;
• To be completely filled;
• State the number of CPD Hours applied for;
• Program to be provided with details of speaker and time allocation for session/s;
• For individual application as presenter/speaker, please submit full paper presented; and
• For individual application as panelist/chairman/facilitator, please indicate so in the form.

The Board will not be held responsible for delay in approval CPD Hours if the above requirements are not meet.
